Job Description

Company Overview:
Family Help Center is a dedicated nonprofit organization committed to supporting families raising children. Since 1973, we have provided vital services, including in-home supports and basic human needs to help families navigate challenging moments and strengthen their bonds.
Job Summary:
This Leadership position is responsible for setting a clear vision, driving strategic initiatives, and delivering measurable results. The successful candidate will demonstrate exceptional decision-making, strong organizational skills, effective communication, and the ability to be a forward thinker to build program sustainability. The Supervisor of Social Care Navigators for our Western New York Integrated Care Coordination (WNYICC) program will oversee a team of Social Care Navigators who connect individuals to essential community resources. The role is pivotal in program development, relationship management, and ensuring the delivery of compassionate, effective support that empowers members and enhances their quality of life. We are seeking a leader who can inspire excellence and foster a culture of trust and accountability, drive operation success, and motivate and empower the team to achieve full potential.
Educational Requirements:
Master's Degree preferred (in Healthcare or Human Services fields). Minimum Bachelor's degree in Healthcare or Human Services .
Experience/Skills Requirements:
3 years of supervisory experience. Supervisory experience in the
WNYICC 1115
medicaid waiver program preferred. 6 months of experience in screening/navigation within a Social Care Network. Extensive proven experience with budgeting and financial planning support, documentation, and compliance reporting in a program to ensure sustainability. Excellent phone and computer skills. Highly organized and skilled with Electronic Health Records (EHR). Knowledge of social work principles, behavioral health practices, and community resource networks. Extremely effective communicator.
Schedule:
Full Time Monday-Friday Day Shift Pay:
$53,000.00 - $55,000.00 per year
