Job Description

The Care Navigator at the Center for Regional Healthcare Innovation (CRHI) at MidHudson Regional Hospital works independently and as a member of the CRHI team in the development and execution of programs and services commencing with the implementation of the New York State Delivery System Reform Incentive Program (DSRIP). The Care Navigator assigned to the Emergency Department fulfills NYS Dept. of Health and DSRIP requirements.
RESPONSIBILITIES
Under the supervision of a
CRHI Vice President:
Performs PPS-related activities associated with care management, including identification of patient's Health Home, Care Management agency and/or behavioral health providers; creating documentation; and adhering to workflows. Ensures that the patient is connected to primary care providers, and other community services, especially for those with multiple chronic conditions and/or uninsured. Coordinates follow-up appointments with community providers. Ensures that discharge plan of care issues are addressed. Accesses electronic health record and PSYCKES and Healthify databases for care transitions/discharge summary purposes. Ensures and supports HIPPA compliance, including obtaining patient consents for information sharing among providers. Ensures referrals to Health Home agencies for eligible patients. Serves as a member of the Clinical Case Management team assigned to the Emergency Department, performing appropriate dept. functions, i.e. attending meetings and completing reports as required. Performs other duties as assigned. Serves as brand ambassador by upholding the WMCHealth Mission, Vision, and CARE values and promoting excellence in the patient experience, during every encounter.
QUALIFICATIONS/REQUIREMENTS EXPERIENCE
Knowledge of healthcare industry and DSRIP preferred Case management experience preferred, and/or experience in emergency dept. and/or working with highly vulnerable populations.

LICENSES/CERTIFICATIONS N/A OTHER
If applicable, the individual performing this job may reasonably anticipate coming into contact with human blood and other potentially infectious materials. Individuals in this position are required to exercise universal precautions, use personal protective equipment and devices, and learn the policies concerning infection control. Experience with Microsoft Office, HMIS training a plus. Project a professional demeanor, able to work independently and as a part of a team. Strong communication skills.
