About the Role We're looking for a dedicated Care Management Assistant to join our team and help us continue delivering the level of care our patients and families deserve. In this role, you'll be a vital part of our administrative team, ensuring excellent service, collaboration, and patient outcomes in a fast-paced healthcare environment. +ADD
SUMMARY OF POSITION
What You'll Do Support RN Case Managers/LMSW in meeting with patient and/or family/personal representative within 48 hours of admission (excluding weekends/holidays), to evaluate, and identify discharge needs in compliance with established guidelines and policies. Provides support and information, as needed. Assists with discharge planning activities to support discharge timely and efficiently through collaboration with providers and multi-disciplinary team. Works to minimize discharge delays and achieve appropriate discharge times. Ensure adherence to confidentiality and privacy policies Supports timely community resource referrals, in alignment with patients' benefits, within EPIC as needed based on patient choice and post-hospitalization needs for discharge and transfer. Provides appropriate linkages, referral coordination, and follow-up. Collaborate with community liaisons and health care partners to facilitate optimal discharge plans. Advocates for the patient's and family's needs. Supports delivering copy of regulatory notices to patients as directed by the RN Case Manager or RN Utilization Manager Issues "Important Medicare Message" (IM) as required 24-48 hours prior to discharges of Medicare and/or Managed Medicare patients. Issues Discharge Notice for Non-Medicare Patients as required within 24 hours to discharge for Commercial, Medicaid and Non-Medicare Patients. Deliver compassionate, patient-centered care in alignment with Saratoga Hospital values Collaborate with interdisciplinary teams to support health, healing, and service excellence Perform administrative responsibilities with a focus on safety, quality, and efficiency Use hospital systems and tools to document care and support operations Continuously seek opportunities to improve processes and support patient and staff satisfaction Serve as a positive, professional representative of our hospital and community What You Bring Minimum of 2 years Acute Care Experience. Knowledgeable of New York State and Federal Entitlement Programs and regulatory requirements.
