JASA GUIDE
Care Navigator supports the delivery of Montefiore's Guiding an Improved Dementia Experience (GUIDE) program in the community as part of a partnership with JASA. The Care Navigator works with traditional Medicare beneficiaries living with dementia and their caregivers to provide comprehensive care navigation, coordination, education, community referrals, and access to respite care. Working independently and as part of an interdisciplinary team, the Care Navigator helps participants and caregivers navigate medical and social service systems and maintain quality of life in the community. Specific Duties and Responsibilities Conduct outreach, enroll eligible beneficiaries, and support ongoing program participation. Complete comprehensive assessments and assist in developing and updating individualized care plans. Provide ongoing care navigation, care coordination, and intensive disease management education. Conduct medication reviews, identify potential medication concerns or barriers, and communicate findings to the clinical team. Educate and support caregivers regarding dementia progression, symptom management, medication adherence, safety, and available resources. Coordinate medical, behavioral health, social service, and community-based supports. Connect participants and caregivers with benefits, home care, respite care, transportation, caregiver support, and other needed services. Conduct home visits and provide services by telephone, virtually, and in clinical or community settings. Monitor participant and caregiver needs and promptly communicate changes or concerns to the interdisciplinary team. Maintain timely and accurate documentation and support program reporting and quality improvement activities. Participate in interdisciplinary team meetings, case conferences, supervision, and required trainings. Perform other related duties as assigned. Required Qualifications International medical graduate. Fluent in English and Spanish. Strong clinical knowledge and ability to conduct medication reviews and provide intensive disease management education and caregiver support. Experience working with older adults, individuals living with dementia, caregivers, or individuals with complex medical and social needs preferred. Strong assessment, care coordination, communication, and problem-solving skills. Ability to work independently and collaboratively as part of an interdisciplinary team. Ability and willingness to conduct home visits throughout the service area (Bronx and Westchester). Strong organizational and documentation skills. Proficiency with electronic health records and standard computer applications.
