If so, NorthCare is looking for someone like you to join our Assertive Community Treatment (ACT) team as a Care Navigator Level 2. This isn't your typical desk job. As a Care Navigator, you'll take mental health services beyond clinic walls, meeting clients where they are, whether it's in their homes, the park, or local service providers' offices. You'll help individuals ranging from challenges to severe mental illness thrive by providing case management, wellness coaching, medication support, rehabilitation services, and care coordination, all tailored to the unique needs of each person.Some Key Duties:
Provide integrated services, case management, rehab, medication prompts/deliveries, and wellness interventions, directly in the community. Coordinate care for physical and behavioral health, ensure follow-through with outside provider appointments, and help clients develop achievable life goals. Assist individuals in accessing housing, employment, education, transportation, and health/disability benefits. Encourage self-management, health education, and advocacy around chronic conditions, stress, smoking cessation, and more. Provide crisis support and intervention as needed, including safety planning and hospitalization coordination. Mentor peers, lead care coordination meetings, and guide complex cases using evidence-based strategies. Transport clients to community resources and appointments, ensuring they never walk the path to recovery alone. Accurately document services, meet productivity standards, and complete required training to maintain certifications.Benefits/Perks:

ODMHSAS Behavioral Health Case Manager II (CM II) Certification ODMHSAS Wellness Coach Certification Two or more years of CMII experience in community mental health Dual certification (CMII/PRSS) preferred Valid Oklahoma driver's license and reliable transportation with business-use insurance Flexibility to provide over 50% of services in community settings Ability to think critically and work independently Proficiency with basic computer systems and documentation CPR, First Aid, and Therapeutic Options certification (or obtained within 30 days) This is a community-based role requiring travel and a high degree of autonomy.
