Job Description

Centerville Clinics, Inc (CCI) is excited to announce that we are now hiring LPNs for our new Care Team Supervisor position our Washington Office! Applicants with strong leadership ability, good organizational and communication skills are encouraged to see more information below: CCI is integrating a new Team Based Care model to our medical offices where our Care Team Supervisor would be assigned to lead a care team for a single provider with a medical assistant and clerical support. The Care Team Supervisor will assure that all team members (provider, MA, Clerical Staff) work as a team and each team member plays an important part to assuring that the team is working together to provide the best care for the patient. The incumbent will also carry out performance improvement initiatives and direct the appropriate level of care using internal and external resource for patients.

Delivers direct and indirect patient care in accordance with Centerville Clinics Inc. Policies and Procedures. Provides education and instruction, within their scope of practice, to patients and families. Work collaboratively with Physicians to achieve desired patient care outcomes. Utilizes clinical skills and education to maintain patient care and will seek assistance from supervisor and physicians when needed Completes assigned administrative tasks; including telephone calls, faxes, and arranges appointments for providers and patients Other duties as assigned
Job Functions:
Conducts pre-visit chart review Assists and coordinates data entry for the disease registry database; ensures patient gaps are met Communicates lab/diagnostic test information to patients and families Contact/follow-up with patients who do not meet their outcome goals Reinforces physician recommendations during appointments and phone outreach Directs practice team and performance improvement structure Maintains patient records and privacy Participates in actively promoting patient health and wellness
Qualifications:
Valid LPN license Experience in medical industry preferred
