Job Description

Job description Remote Patient Devices LLC is seeking a highly organized and detail-oriented Care Coordinator to assist patients in maintaining and improving their health. You will act as the liaison between patients and their providers, by monitoring patient data, and speaking with patients by phone.
Conversations include:
Reviewing device readings. Educating patients on improving their symptoms. Supporting patients with their plan of care established by their provider.
Principal Duties:
Verify and record the patient's demographics, medical issues, medications and risk management factors to create a clinical summary. Create a patient-centered, comprehensive plan of care for all health issues affecting them. Review and evaluate readings. Communicate with practices, based on established protocols, when readings exceed established parameters. Assist patients in communicating with their providers. Ensure continuity of care by setting and reminding patients of regularly scheduled visits.
Requirements:
Certified and Active:
CNA, CMA, RMA, CCMA, or LPN. Must be available for 4 weeks of in-person training in our Downers Grove office. Ability to follow HIPPA guidelines. Handle potentially difficult conversations calmly and professionally. Previous F2F healthcare experience required. Previous phone experience preferred. Bilingual candidates a plus.
