Home Office / In-clinic visits Support patients directly through outreach, access, and care navigation We're hiring a Healthcare Liaison to support outreach, enrollment, and engagement for our Enhanced Care Management (ECM) program serving Medi-Cal members with complex needs. This role is ideal for someone who enjoys working directly with patients in the field , building relationships, and helping individuals overcome barriers to care. You will play a key role in connecting patients to services, supporting care plans, and ensuring coordinated, whole-person care. This is a highly active, community-based role with significant travel throughout the region. Who we are looking for Experience working directly with patients in healthcare, outreach, or community-based roles Strong interpersonal and communication skills with the ability to build trust and engage patients Experience working with Medi-Cal, underserved populations, or individuals with complex needs preferred Ability to support patients in navigating social services and community resources Comfortable working in a field-based role , including home visits and community outreach Organized and detail-oriented with the ability to manage multiple patients and follow-up activities Strong collaboration skills and ability to communicate across care teams, providers, and community organizationsWhat You'll Do:
Outreach & Enrollment Identify and outreach to eligible patients for enrollment in the ECM program Educate patients on available services, benefits, and care support Build relationships to encourage participation and engagement Patient Engagement & Care Coordination Serve as a patient advocate , helping individuals navigate healthcare and social services Support patients in following care plans and accessing needed resources Communicate regularly with the Social Case Manager and care team regarding patient needs and barriers Help coordinate care to improve outcomes and reduce barriers Community & Resource Navigation Connect patients to community-based organizations and support services Build and maintain relationships with community partners and healthcare providers Support efforts to reduce health disparities and improve access to care Documentation & Collaboration Document all outreach, patient interactions, and care coordination activities Track patient engagement and follow-up to ensure continuity of care Collaborate with internal teams, health centers, and managed care partnersQualifications:
Required High School Diploma or GED Strong communication and interpersonal skills Ability to work in a fast-paced, field-based environment Valid driver's license, active insurance, and reliable transportation Preferred Experience in healthcare outreach, care coordination, or community health Experience working with Medi-Cal, FQHCs, or underserved populations Knowledge of social determinants of health (SDOH) and community resources CPR certificationWork Environment & Requirements:
Hybrid role with up to 80% local travel Field-based work including home visits and community outreach Must reside in or be able to travel throughout San Diego County Must be able to work occasional evenings and weekends as neededCompliance & Confidentiality:
