As a member of the Health Support Center (HSC) team, you'll support those in our facilities who are interfacing and providing care to our patients and community members — positively impacting our mission of making communities healthier®. More about our team Our Population Health team partners with providers, care managers, and community organizations to improve health outcomes across the Clinically Integrated Network (CIN). We focus on patient engagement, advocacy, and connecting individuals to both medical and community resources to address clinical and social needs. How you'll contribute A Care Navigator / Community Navigator who excels in this role: Assists patients within the network who are high or rising risk and eligible for additional healthcare support and services. Acts as a patient advocate and navigator, conducting comprehensive and preventive screenings and supporting engagement in care. Connects patients with network providers, facilities, and resources such as prescription assistance and durable medical equipment (DME). Facilitates clear communication of the patient care plan among the care team, resource affiliates, patients, and families. Coordinates continuity of care across healthcare settings (inpatient, outpatient, skilled care, hospice, home health, etc.) to ensure appropriate utilization of clinical resources. Works collaboratively with primary care practices to enhance quality and efficiency in patient care and utilization of services. Addresses social determinants of health by linking patients to community-based programs and services. Engages effectively with diverse populations to reduce cultural and socio-economic barriers to care. Performs other duties as assigned. Ongoing learning and career advancement opportunities. What we're looking for Applicants should have a high school diploma or equivalent (Medical Assistant or higher preferred) and two years of experience in an ambulatory healthcare setting.Additional qualifications include:
Experience in population health initiatives such as chronic disease management, care management, or utilization management. Excellent oral and written communication skills. Strong customer service and leadership abilities. Ability to work independently and prioritize effectively. Team-oriented mindset with strong collaboration skills. Highly organized and detail-oriented. Proficiency in Microsoft Office (Outlook, Word, Excel, Teams). Knowledge of motivational interviewing, teach-back methods, and self-management support strategies.Travel Requirement:
