Summary:
Coordinates the patient's cardiovascular care to increase the efficiency and quality of care experience that is delivered to the patient. The navigator will be a partner with the patient starting at the time of the patient's diagnosis/scheduling of initial appointment and will continue through the patient's journey with our Cardiovascular Service Line. The navigator will be the patient advocate and provide care coordination in a highly supportive and personal manner. Serves as a liaison to ensure that patients' and families' support systems receive resources and services as needed. Ensures and coordinates (where appropriate) new patient referrals, scheduling of exams, procedures and appointments. Coordinates care for patients returning to their communities for treatment and follow-up. Works with appropriate teams to help resolve any health-related social need. Educates and provides information to new and existing patients and families regarding work-up, procedures, surgery, therapy, support services, financial issues, community resources and follow-up. The navigator will focus on answering all patient and caregiver questions, ensuring a seamless experience of care and delivering excellent customer service. Will serve as a liaison for patients and families between departments. Also facilitates appropriate clinical information flow to and from referring clinicians. This team members will conduct service recovery when needed and will be responsible for patient satisfaction. The Cardiovascular Navigator is a patient-facing and referring clinician-facing role that facilitates interdisciplinary communication within the care continuum to promote continuity of care and delivery of exceptional patient experience.
Requirements:
Graduate of accredited School of Nursing and must be licensed as a Licensed Practice Nurse (LPN) in the appropriate state(s) to practice nursing. Minimum of two years of experience, particularly in Cardiovascular service, preferred.
Skills:
The successful candidate will have excellent communication and interpersonal skills. He or she will be exceptionally patient-focused and will exhibit a drive to address all patient issues. Additionally, this person will be a natural collaborator and will be able to easily work with multiple different teams.

Valid and active LPN licensure in appropriate state(s)CPR required
