Job Description
LifePoint Health is seeking a Clinical Liaison to connect patients, families, and providers with our community-based hospitals and rehabilitation services. In this mission-driven role, you will build relationships with physicians, case managers, and community partners, educate them on our clinical programs, and coordinate smooth transitions of care. Responsibilities include assessing patient suitability for services, facilitating referrals and admissions, and maintaining accurate referral data. You'll collaborate closely with clinical teams to support quality outcomes and patient satisfaction while helping expand access to needed medical services in the communities we serve.
