As a community health liaison, you will build working relationships, solve problems and support patients while they learn to self-navigate the health care system. CHL's will work closely with medical providers, primary care teams and other agencies to improve patient care and outcomes. The goal of this program is to help patients achieve independence in managing their own healthcare needs. He/she will conduct interviews and basic clinical evaluations of each patient to determine their clinical and social needs. He/she will teach patients how to properly manage their chronic illnesses and resolve acute medical conditions through health education and patient empowerment.
Key Duties Patient Navigation & Support:
Identify patient's understanding of medical care/outcome and provides appropriate education and resources to address needs. Coach patients in effective management of their chronic health conditions and self-care. Help patients develop health management plans and goals. Follow-up with health management/care plans with both patients and providers. Facilitate communication and coordinate services between providers. Assist and educate patients with navigation of the health care system (i.e. scheduling and keeping appointments, translation, pharmacy programs, transportation arrangements, communicating with providers, etc.) Assisting clients with SSI/Medicaid applications and navigation. Provide translation support when needed during travel and outreach. Link patients to available public resources (i.e. government assistance, local health programs, pharmacy assistance programs, etc.) Teach patients how to find and utilize public resources independently. Ensure patient understanding and compliance with provider care plans. Establish positive, supportive relationships with clients and provide feedback. Work closely with Patient Access Coordinator and Program Manager to identify patient needs.
Administration:
Manage incoming calls and greeting visitors with a professional and courteous demeanor Maintain accurate patient records; ensure files are accurate and up to date. Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place. Promote PACC's mission statement, value statement and philosophy with staff members, patients, families, community representatives and healthcare partners. Provide back-up coverage to PACC staff when necessary. Other duties or special projects as assigned.
Requirements:
Any combination of 2 years health/social services experience and/or education. Highly professional; able to use good judgment and maintain a high level of confidentiality and sensitivity. Strong communication, public speaking, and interpersonal skills. Ability to work independently with support and supervision from Program Manager and to also collaboratively and communicate effectively with others, including patients and families, staff, and community health care providers. Ability to demonstrate resourcefulness, patience, good judgement in emergency situations. Proficient in organizing and prioritizing workloads to meet deadlines. Basic understanding of medical terminology. Proficiency in Microsoft Office 365 preferred. Bilingual/bicultural (Spanish) Knowledge of CareScope a plus. Strong knowledge of community resources preferred. Current driver's license, verifiable good driving record and reliable transportation. Must be able to work occasional flexible hours as requested.
