This role supports both regional partners and patients by coordinating with internal and external stakeholders. They assist patients at every stage of care and follow up with patients, providers, and partners to ensure all needs are addressed efficiently. Additionally, the navigator acts as a liaison between the health system and providers, patients, families, and the community, helping to deliver outstanding service and seamless continuity of care.
