Position Summary:
We are seeking a compassionate, organized, and patient-focused Specialty Services Patient Navigator to join our healthcare team. The Patient Navigator plays a critical role in guiding patients and families through the healthcare continuum by coordinating specialty care services, facilitating communication between providers and patients, and ensuring a positive patient experience. The ideal candidate will have experience in behavioral health, outpatient services, patient coordination, or hospital-based care and possess excellent communication, organizational, and problem-solving skills. This position requires the ability to work collaboratively with interdisciplinary healthcare teams while maintaining professionalism, confidentiality, and compliance with healthcare regulations.
Key Responsibilities:
Serve as a liaison between patients, families, physicians, and healthcare teams to ensure continuity of care and effective communication. Assist patients in navigating healthcare systems, specialty services, referrals, appointments, treatment plans, and follow-up care. Coordinate patient scheduling, referrals, discharge planning, and access to community resources and support services. Educate patients and families regarding healthcare services, treatment options, patient rights, and discharge instructions. Address patient concerns and assist in resolving issues promptly and professionally to promote patient satisfaction. Maintain accurate and timely clinical documentation, patient records, and communication logs in accordance with HIPAA and organizational standards. Collaborate with physicians, nursing staff, behavioral health professionals, referral sources, and community organizations to support patient care needs. Support patients with behavioral health and mental health service coordination when appropriate. Ensure compliance with all healthcare regulations, safety protocols, and confidentiality requirements. Participate in quality improvement initiatives and contribute to maintaining high standards of patient care and service excellence. Assist with insurance coordination, referrals, and resource management to improve patient access to care. Perform additional duties as assigned to support departmental and organizational goals.
Education & Experience:
Bachelor's degree in Healthcare Administration, Biology, Psychology, Social Work, Nursing, or a related healthcare field preferred. Equivalent healthcare experience may be considered in lieu of degree completion. Minimum of 2 years of experience in patient care coordination, patient advocacy, behavioral health, specialty services, or healthcare administration preferred. Experience working in outpatient, hospital, psychiatric, or specialty healthcare settings strongly preferred. Familiarity with medical terminology is a plus. Active CPR certification is required. Basic Life Support (BLS) certification required or obtained within designated timeframe. Nonviolent Crisis Intervention certification preferred. ACLS certification preferred.

Physical Requirements:
Ability to alternate between sitting and standing for extended periods. Capable of lifting, holding, pushing, and pulling up to 15 pounds. Proficient in operating a computer or laptop, including extended periods of typing, clicking, and monitor use. Adept visual capabilities including close vision, color distinction, depth perception, and the ability to adjust focus with strong hand-eye coordination. Effective verbal and written communication skills to interact with clients, team members, and customers. Ability to read, write, and comprehend information through active listening. Able to consistently, safely, and successfully perform essential job functions in compliance with ADA, FMLA, and applicable federal, state, and local standards, including productivity expectations. Dependable attendance and punctuality in alignment with regulatory and organizational standards.
