Worker Sub Type Regular Work Shift First Shift (United States of America) Pay Grade 135 Job Description The Heart Failure (HF) Navigator functions as an advocate for patients diagnosed with Heart Failure. The primary job function is to act as an expert on Heart Failure services and a liaison between our patients, providers and ancillary services in both the inpatient and outpatient settings. The HF navigator analyzes information and take actions for facilitating health and wellness of the patient. This role nurtures working relationships with medical director, physicians, nursing staff, and other disciplines involved in treatment or support of patients. The following is a summary of the major duties of this role. Other duties may be performed, which are not mentioned below. Specific activities may change from time to time.
Completes patient information analysis:
Collaborates with Patient/Provider and prioritizes patient needs, problems and goals; intervenes or makes referral Evaluates, compares and interprets ambiguous/changing patient data Explores patient's understanding and knowledge of current health status Performs patient observation and monitoring: Applies nursing judgment to determine level of care assigned or delegated Coordinates scheduling and facilitation of testing, appointments with providers: Applies specialized expertise to solve problems, generate ideas, plan care, teach and navigate patient through new services Navigates development of multiple health care partnerships to achieve a positive health impact on the patient Promotes patient wellness and facilitates healing: Anticipates future needs and educates or refers to valid sources of information. Understands the patient's unique perspective by assuring the right education, time, and environment for learning Evaluates outcome of learning Coordinates care processes for patients requiring multiple services. Serves as a liaison for patients and providers. Acts as an active listener for concerned patients receiving diagnostic testing Encourages team environment through navigation processes Promotes decreased readmission by creating an environment that provides access to care. Instills confidence in the patient's own ability to regain/optimize their own health. Assists and participates in quality improvement and accreditation projects. Education Bachelor's in Nursing (BSN) required. Master's in Nursing (MSN) preferred. Experience 3-5 years of cardiac experience preferred. Certification & Licensure Registered Nurse with current Virginia license required. Heart failure certification preferred. BLS Certification (Basic Life Support) - American Heart 'Healthcare Provider' (HCP) - AHA approved required. Qualifications Excellent verbal and written communication skills required. Ability to encourage team environment through navigation processes required. Ability to provide education required. Ability to act as liaison between our patients, providers and ancillary services in both the inpatient and outpatient settings required.
