Job Description
Trios Health in Kennewick, WA is seeking a Nurse Navigator to guide patients and families through complex care journeys, with emphasis on oncology and chronic conditions. In this role, you will coordinate appointments, referrals, and follow-up care, provide education on diagnoses and treatment options, and serve as a consistent point of contact across the care continuum. You will collaborate closely with physicians and multidisciplinary teams, advocate for patient needs, and support Trios Health's culture of compassion, excellence, safety, and teamwork while contributing to quality improvement and outstanding patient experiences.
