We're looking for a compassionate, relationship-driven Clinical Liaison to join the Evergreen Healthcare Group team! In this regional role, you'll serve as the key connection between hospitals, families, and our Seattle facilities — helping patients transition to post-acute care with confidence and clarity.

Key Responsibilities:

Develop and maintain relationships with hospitals, physicians, discharge planners, and case managers Evaluate patient referrals and assist with clinical screenings and pre-admission assessments Coordinate admissions and ensure timely, efficient transitions from acute to post-acute care Serve as a clinical resource and liaison to families and referral sources Collaborate with internal facility teams to align care plans and meet resident needs Track referral activity and participate in census development initiatives

Qualifications:
