JOB OBJECTIVE
The RN Care Navigator will serve as the clinical hub of the Promega Wellness Center, providing proactive care coordination, clinical triage, and patient navigation under the direction of the Medical Director. This role bridges direct patient care with operational support, ensuring employees receive timely, coordinated, and clinically informed guidance aligned with the Wellness Center's vision for whole-person integrated care.
CORE DUTIES
Care Coordination & Navigation 1. Manage clinical inbox workflows including provider message triage, results communication, referral coordination, and follow-up documentation. 2. Serve as the first clinical point of contact for employee inquiries, providing evidence-based guidance, clinical assessment, and appropriate routing to Wellness Center providers or external resources. 3. Coordinate care transitions between the Wellness Center, primary care physicians, specialists, and other healthcare providers to ensure continuity of employee care. 4. Facilitate timely scheduling of appointments based on clinical urgency and provider availability, applying triage protocols to appropriately prioritize employee needs. 5. Proactively follow up with employees following clinical visits, procedures, lab results, or referrals to ensure care plans are understood and actioned. Clinical Assessment & Triage 6. Conduct clinical telephone and in-person triage assessments for employees contacting the Wellness Center, applying clinical judgment to determine appropriate level of care. 7. Perform and document nursing assessments, vital signs, and screenings as part of clinical workflows. 8. Provide clinical education and self-management support to employees for chronic conditions, preventive care, and health optimization goals. 9. Administer vaccinations, perform phlebotomy, and support biometric screening events and health risk assessments (HRA). 10. Assist providers with clinical procedures and patient care as needed. Operational & Administrative Support 11. Maintain accurate, timely documentation of all clinical interactions and care coordination activities in the electronic medical record (EMR). 12. Collaborate with Wellness Center providers (Medical Director, PA-Cs) to support efficient clinical workflows and ensure high-quality, proactive patient care. 13. Support work injury management including coordination with Human Resources and EH&S to ensure appropriate Workers' Compensation protocols are followed. 14. Serve as a clinical liaison and employee advocate to facilitate communication with external treating physicians and care teams. 15. Participate in wellness program delivery including vaccination clinics, annual screenings, and population health initiatives. 16. Maintain compliance with HIPAA, annual clinical training requirements, and CPR/AED certification. 17. Support Research and Development departments with clinical or sample-collection needs as requested. 18. Other duties as directed by the Medical Director. 19. KEY QUALIFICATIONS
1. Bachelor's degree in Nursing (BSN) or Associate's degree with equivalent clinical experience. 2. Current RN licensure in the State of Wisconsin. 3. Minimum 3 years of clinical nursing experience; experience in ambulatory, occupational health, or primary care settings strongly preferred. 4. Current CPR/AED certification. 5. Demonstrated experience with clinical triage and care coordination. 6. Proficiency in EMR documentation and clinical inbox management. 7. Exceptional organizational skills and ability to manage multiple competing priorities. 8. Strong interpersonal and communication skills with ability to build trust as a clinical resource for employees. 9. Ability to work independently and exercise sound clinical judgment within scope of practice. 10. Commitment to patient advocacy, confidentiality, and professional excellence. 11. Demonstrated proficiency in digital tools and technology, including Microsoft applications and mobile platforms, with the ability to adapt and leverage emerging technologies, such as artificial intelligence, to improve efficiency, automation, and collaboration.
PREFERRED QUALIFICATIONS
1. Experience building clinic protocols and experience in operational quality improvement strategies. 2. Familiarity with health optimization, precision health, or functional medicine principles. 3. Training or certification in health coaching. 4. Phlebotomy proficiency.
PHYSICAL DEMANDS
1. Ability to lift 30 lbs. as needed. 2. Ability to use standard office and clinical equipment. 3. Ability to stand for extended periods during clinical and screening activities.
