Correct Rx Pharmacy Services, Inc., a nationally recognized leader in institutional pharmacy, is seeking a Clinical Pharmacist for our Adelanto, California location. Our clinical pharmacists play an integral role on our team, contributing to medication care plan development, direct patient care clinic services, implementation of innovative clinical programs, medication therapy outcome monitoring, and operational oversight to ensure medications are ordered and administered to patients accurately and efficiently. This is an excellent entry-level opportunity for pharmacists seeking a unique and rewarding clinical practice setting.
Responsibilities and Duties:
  • Evaluate, monitor, and enhance medication therapy and care planning
  • Participate in Pharmacy & Therapeutics (P&T) Committee meetings
  • Conduct complex Drug Utilization Review and analysis
  • Support Disease State Management programs
  • Provide Drug Information services
  • Contribute to Formulary Management
  • Perform High Acuity Patient Case Reviews
  • Participate in ongoing administrative meetings
  • Deliver Patient Education
  • Provide Provider Consultations and Education
