Position Overview:
The Oncology Pharmacist will ensure safe, optimal and efficacious drug therapy for patients, as you review, process and monitor physician orders and patient medication profiles. As an active member of the health care team, the Oncology Pharmacist is responsible for contacting other health care professionals as appropriate, performing duties and responsibilities in a manner consistent with the hospital's mission and values.
What You'll Do:
Dispense and prepare specialized medications for pediatric oncology and hematology patients and collaborate across disciplines to shape and execute patient treatment plans. Verify medication orders for accuracy, appropriateness, and potential interactions and educate and counsel patients, families, and caregivers on medication use Monitor drug therapy for effectiveness and adverse effects. Contribute to patient care through active participation in clinical rounds. Manage chemotherapy protocols and safe handling of hazardous drugs. Perform medication reconciliation during care transitions and provide expert drug information and consultations. Support the development of pharmacy policies and procedures and ensure adherence to all regulations and safety standards. Engage in quality improvement and research initiatives. Maintain precise patient and medication records. Stay current with advancements in pediatric hematology/oncology pharmacotherapy. Mentor pharmacy students and residents
What You'll Bring:
License:
Active and in-good-standing
California State Pharmacist License Education:
Pharm D or B.S.
Experience:
1+ years of clinical experience required, 1+ years of oncology experience required (pediatrics oncology experience preferred), 1+ years of pediatrics experience preferred
Screening:
Must pass a background check, drug screen, and health screening
