care Patient treatment Patient education Diabetes care Vaccination administration Full Job Description A Clinical Pharmacist- Multisite contributes to AHF by expanding primary care functions of pharmacists at more than one site to provide individualized health promotion and disease prevention, including administration of vaccines, long-acting HIV treatments, diabetes monitoring, nicotine reduction programs and other subject matters, in collaboration with other AHF providers. - Multisite will work as a part of a close-knit team to provide individualized health promotion and disease prevention, including administration of immunizations, diabetes and smoking cessation programs, as well as other Subject Matter Expertise (SME) programs as developed by AHF, with appropriate collaborative drug therapy management authority.
