Abdominal Transplant Administration Hoag Health System - 4.0 Newport Beach, CA Job Details Full-time 8 hours ago Qualifications Registered Pharmacist (RPh) Inpatient BLS Certification Licensed Pharmacist (State-Specific) Acute care facility experience Doctor of Pharmacy Licensed Pharmacist Full Job Description The Clinical Pharmacist Specialist, Transplant Services provides pharmaceutical services to the hospital's patients, leading in the delivery of pharmaceutical care for transplant recipients. Provides pre- and post-transplant clinical pharmacy services. All associated areas may be included as well, including transferring units, procedural areas, and outpatient clinics. The Clinical Pharmacist Specialist, Transplant Services supports pharmaceutical services for their areas through active participation in patient care rounds with multidisciplinary teams, medication profile review, and development of optimal drug protocols and guidelines. Consults, evaluates, directs, and monitors specific drug therapies, working in cooperation with physicians and all staff in their areas. Educates patients, caregivers, and other healthcare staff and students, is responsible for the development and maintenance of all pharmaceutical care services for their designated areas, and collaborates with all relevant staff to develop new services or improve medication utilization for formulary items specific to their areas. Participates in quality improvement and related committees. Projects an image of professionalism in communication, appearance and conduct. Maintains a working knowledge of departmental standard operating procedures. Performs other duties as assigned.
Education and Experience:
Graduation from an ACPE-accredited pharmacy school program with at least a Doctor of Pharmacy degree At least three years' experience in acute care pharmacy services in a progressive hospital setting PGY1 pharmacy residency training, and PGY2 pharmacy residency training or fellowship in Transplant Pharmacy are preferred Current/Recent working experience in Transplant Pharmacy is preferred Fulfills mandatory stroke education requirements per certification agency Licenses Required Valid license to practice pharmacy in the State of California Licenses Preferred N/A Certifications Required Current BLS (AHA) certification upon hire Certifications Preferred Board Certified Pharmacotherapy Specialist (BCTXP)
