Dispensing Pharmacist Gojji - 1.0 San Bernardino, CA Job Details Full-time $130,000 - $160,000 a year 5 hours ago Benefits Health savings account Health insurance Dental insurance Flexible spending account Paid time off Parental leave Vision insurance Life insurance Referral program Qualifications CPR Certification Teamwork Phone communication First aid Licensed Pharmacist (State-Specific) Doctor of Pharmacy First Aid Certification Licensed Pharmacist Organizational skills Typing Full Job Description A Dispensing Pharmacist is primarily responsible for ensuring the accuracy and appropriateness of dispensed prescription orders (drugs and durable medical supplies) and providing clinical care, while upholding federal and state laws, regulations, and standards. Secondarily, providing comprehensive disease management that includes disease state monitoring, medication review and patient education. Responsibilities Strong drug information background and ability to reference drug information to ensure patient drug effectiveness and safety. Monitor adverse drug reactions, drug-drug interactions, drug-disease interactions, drug-food interactions, and contraindications and work with providers to optimize drug regimen Educate and counsel patients on the use of newly prescribed drugs and/or existing drugs Educate health care providers on formulary updates, new medications, and therapy recommendations (e.g. drug interactions, dose adjustments, alternative therapies, adverse reactions, contraindications) Strong clinical background and knowledge of chronic disease states such as diabetes, hypertension, cardiac diseases, asthma/COPD, and more. Monitor patients for needed updated labs (e.g. Hemoglobin A1C, Fasting Lipid Panel, Comprehensive Metabolic Panel, Urine Microalbumin) and/or abnormal labs (e.g. elevated/low potassium, elevated liver function tests, elevated serum creatinine) Monitor patients for medication compliance and address issues with non-compliance Conduct patient assessments for chronic disease states, implement and monitor patient care plans and work under provider collaborative practice agreement protocol Provide preceptorship and leadership to pharmacy resident(s) and student(s) Other duties as assigned
Qualifications License:
Maintain a valid, clear, and active pharmacist license with the California State Board of Pharmacy (BOP)Education:
Doctor of Pharmacy Degree (PharmD)Continuing Education:
Complete 30 continuing education (CE) credits every 2 years At least 15 of the 30 units of CEs are diabetes-related (preferred) and mandatory requirements from the California State Board of Pharmacy (BOP)Other Company-Specific Qualifications:
Spanish language speaking (preferred) CPR and First Aid Certified (preferred) Proficient in typing and using computer programs such as Excel and Word and various customer relation platforms Clinically sound with the ability to provide telephone-based patient education Skills Proven experience as a pharmacist Thorough understanding of dosage administration and measurement, chemical compounds, medical brands etc. Outstanding knowledge of MS Office and Pharmacy information systems (eg. PrimeRx) Good organizing skills Excellent communication skills with a customer oriented approach There are prolonged periods of sitting, standing and working at a computer and on the phone with a headset.Job Type:
