SUMMARY OF POSITION
The Pharmacist will provide pharmacy services to patients and health providers. Services including but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing and recording drug information, and patient information in the system. Ensure compliance with Federal, State and local laws and regulations.
ESSENTIAL JOB FUNCTIONS
Applies knowledge of medications and their indications, contraindications, dosing, side effects, and proper administration. Reviews prescriptions for appropriateness and contacts the prescriber for any medication related problems or order clarification. Prepares and/or oversees the preparation, labeling, verification of appropriate patient, and dispensing of medications in accordance with provider's prescriptions to patients. Advises and educates patients and/or other health professionals on drug indications, contraindications, dosage, medications, interactions, medication formulary, and side effects; provides other drug information as appropriate. Administers vaccinations to patients per protocol and maintains vaccine training compliance Oversees the acquisition and disbursement of medications to clinic sites and offices. Follow policies and procedures on storage and recordkeeping systems to provide for safekeeping of pharmaceuticals with particular attention to controlled substances. Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental and/or infection control standards. Responsible for the efficacy, potency, and safety of all pharmaceutical dispensed and/or used within the pharmacy. Monitors drug and/or nutritional therapy of patients to detect adverse drug reactions and to ensure optimal, efficacious and cost-effective therapy. In the absence of the PIC or when assigned by the PIC, provides leadership and functional direction to pharmacy interns, students, pharmacy technicians, and other pharmacy personnel by offering suggestions and answering questions; resolve professional, technical, and clerical problems; monitor and manage pharmacy workflow; orient new employees to assigned duties; train pharmacy team in new procedures and techniques; ensure adherence to established pharmacy policies and procedures, state and federal guidelines. Accepts the responsibility for problem resolution, e.g. resolving professional problems related to the interpretation of medication ordered, drug furnishing irregularities, service complaints, and technical problems such as equipment or drug procurement issues. Must be able to cover all Pharmacy locations. Performs other job duties as required by the manager/Director.
QUALIFICATIONS
Technical or academic knowledge: Academic training as a Pharmacist. Knowledge of basics of clinical medicine. Valid license as a Pharmacist from the California State Board of Pharmacy Current documentation of Basic Life Support required
LANGUAGE
Must be able to fluently speak, read and write English. Fluent in Chinese (Cantonese and/or Mandarin), Vietnamese, Spanish and other languages may be required depending on location. Fluency in other languages are an asset.
STATUS:
This is an FLSA non-exempt position. This is an OSHA high-risk position.
