Hospital Pharmacy Responsibilities:
  • Interpret medication orders and accurately transcribe and maintain computerized patient medication profiles Accurately compound and dispense pharmaceuticals, including IV admixture preparations, chemotherapy, and parenteral nutrition products
  • Monitor drug therapy regimens for contraindications, drug interactions, drug food interactions, allergies, and appropriateness of drug and dose
  • Perform pharmacokinetic drug dosing per hospital protocol
  • Review and interpret culture and sensitivity data for antibiotic appropriateness and recommend changes as needed
  • Accurately read, extract, and interpret patient chart information
  • Detect and report adverse drug reactions accurately and in a timely manner
  • Sustain the formulary by minimizing non-formulary procurements and promoting rational drug therapy selection
  • Provide clinical consultation and clarification to practitioners and suggest cost-effective therapeutic alternatives as needed
  • Provide drug information to patients and families as needed
  • Issue controlled substances to patient care areas and maintain records as required by law
  • Accurately document all clinical activities and interventions
  • Participate in quality improvement and medication review activities
  • Collect data, conduct quality monitors and inspections, and maintain logs and records as assigned
  • Participate in the development and presentation of orientation, education, and training programs for pharmacy, nursing, and medical staff as needed
  • Serve as a member of the Code Response Team Experience
  • Inpatient hospital pharmacy experience required
  • IV admixture and sterile compounding experience required
  • Chemotherapy and parenteral nutrition preparation experience preferred
  • Pharmacokinetics and antibiotic stewardship experience preferred Skills
  • Strong clinical judgment across a wide range of inpatient pharmacy functions
  • Effective communicator with physicians, nurses, and multidisciplinary care teams
  • Detail-oriented with accurate documentation and chart interpretation skills
  • Comfortable rotating shifts in a fast-paced hospital environment
  • Calm and confident as a member of the Code Response Team Qualifications
  • An active Colorado Pharmacist License in good standing is required
  • BLS certification through the American Heart Association is required
  • ACLS certification through the American Heart Association is required
  • Graduate of an ACPE-accredited School of Pharmacy with a BS in Pharmacy or PharmD required
  • Proficiency in EPIC required Education
