Full Job Description Overview:

1. Resolves drug distribution problems assuring accurate and timely dose delivery to all patients 1.1 Responds to telephone requests for medication in appropriate fashion accommodating callers request or routing call to appropriate person. 2. Interprets Clinical and Pharmacokinetic Lab Data in order to formulate medication monitoring plans. 2.1 Demonstrates knowledge of basic pharmacokinetic principles. 3. Problem Identification & Resolution 3.1 Communicates and documents problem detection, relevant findings, recommendations, and necessary follow-up. 4. Provides drug information to nurses, MDs, and other health care professionals 4.1 Serves as primary source of drug information to physicians, nurses, and other health care professionals 5. Educational Activities 5.1 Participates in and contributes to educational forums within the department (written or verbal). 6. Documents medication use variances (MUVs) and Adverse Drug Reactions (ADRs). 6.1 Provides vialble solutions to prevent recurrence of MUVs 7. Regulatory and Safety Quality Compliance 7.1 Effectively demonstrates the principles and conduct consistent with a
High Reliability Organization Qualifications:
EDUCATION
Graduation from an accredited College of Pharmacy with a B.S. or Pharm.

D. degree. Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.
EXPERIENCE
Two years experience in an acute care hospital pharmacy is preferred for this position.
LICENSURE
Licensed to practice pharmacy in Connecticut (or eligible for reciprocation)
SPECIAL SKILLS
This individual should demonstrate knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution, and formulary management. Excellent communication skills and the ability to work with others are required.
