Category Pharmacy - Pharmacist Job Type Contract A PRN pharmacist role is available to provide support at a 34-bed long-term acute care hospital (LTACH) focused on critical illness recovery. This position emphasizes independent clinical practice within an inpatient setting and involves medication distribution, sterile product preparation, clinical consultation, and regulatory adherence.
Key responsibilities include:
Completing accurate medication reconciliation and developing prior-to-admission home medication lists using multiple information sources Delivering discharge medication counseling to patients and families Monitoring drug therapy for contraindications, interactions, allergies, and appropriate dosing Providing clinical recommendations to the medical team Participating in antimicrobial stewardship, interpreting culture results and guiding infection-related therapy Performing pharmacokinetic and anticoagulation dosing and monitoring Maintaining medication inventory, preparing and verifying medications per standards, and managing controlled substance documentation Supporting medication-use review, quality assurance, and Pharmacy & Therapeutics Committee activities Ensuring compliance with Federal and State pharmacy laws, JCAHO standards, and facility policies Educating staff, other healthcare professionals, and patients as appropriate Advocating safety policies and promoting a positive work environment
Qualifications:
Active pharmacist license valid for this assignment Hospital or inpatient pharmacy experience preferred EPIC electronic medical record proficiency Strong clinical knowledge and communication skills Ability to work independently in an LTACH setting Availability for PRN daytime shifts Experience with medication reconciliation, antimicrobial stewardship, pharmacokinetics, and anticoagulation management
