Pharmacy :

Provides approprie selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome. Provides pient-specific therapeutic drug monitoring and communices relevant findings and/or recommendions of other health care providers in charge of the pient both orally and in writing. Performs continuous evaluion of prescribed medicions to assure optimal drug therapy. provides educion counseling to pients. Completes health and medicion histories including pient interviews. Assess drug safety and efficacy, including evaluion of physical symptoms. Recommends formulary alternives and completes non-formulary consults as assigned. Reviews all medicion orders for approprieness, drug selection, dosage, route of administrion and the amount, prior to dispensing. Ensures all medicion orders are entered into the electronic pient medical record. Processes, checks and fills all orders in a timely fashion. Review of the pient's allergy history, drug-drug, drug-nutrient, drug-laborory, and drug-disease ste interactions and assessment for clinical relevance should be performed and dealt with appropriely. Reviews medicion profiles and monitors for compliance or potential abuse. Provides leadership in competency development, educion, and documention for areas of anticoagulion. Ensures all facility mandory trainings are accomplished within established timetables and properly documents in service trainings files. Supervises clinical pharmacy specialists and clinical pharmacists in the provision of clinical, long-term care and operions pharmacy services. Establishes work schedules and alloces staff as needed to provide adeque coverage. Maintains time and tendance records and approves leave. Ensures all protocols, procedures, and policies are current and reflective of evidence-based practice. Other duties as assigned.
Work Schedule :
Generally an 8-hour tour Monday through Friday between the hours of 7:00am

Employees may be required to work days, evenings, nights, weekends, and holidays on roting schedule. The hours may vary per the needs of the service.

English Language Proficiency :
Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Educion :
(1) Gradue of an Accredition Council for Pharmacy Educion (ACPE) accredited College or School of Pharmacy with a baccalauree degree in pharmacy (BS Pharmacy) and/or Doctor of Pharmacy (Pharm.

D.) degree.
NOTE:
Prior to 2005 ACPE accredited both baccalauree and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy. (2) Gradue of foreign pharmacy degree meet the educional requirements if the gradue is able to provide proof of achieving the Foreign Pharmacy Gradue Examinion Commission (FPGEC) Certificion, which includes passing the Foreign Pharmacy Gradue Equivalency Examinion (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure :
Full, current, and unrestricted license to practice pharmacy in a Ste, Territory, Commonwealth of the United Stes (i.e. Puerto Rico), or the District of Columbia. Must maintain current registrion. Pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probionary stus may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
NOTE:
Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential terminion of such license by the Ste for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of th de, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocions or voluntary surrenders of Ste license, registrion or certificion.

Pharmacy Supervisor:
Experience :
You must have least 1 year of experience equivalent to the GS-12 grade level. Examples of this includes, but is not limited to: reviewing, interpreting, and verifying medicion orders for approprieness; processing and filling medicion orders; interacting with and making recommendions to other clinical staff regarding medicion therapy ordered to ensure safe and effective care; reviewing the pient's medicions, allergies, labs, and other pertinent informion from the medical record to identify and solve medicion-reled problems; contacting providers as approprie; documenting recommendions and interventions.
Preferred Experience:
Prior supervisory or program management experience. Completion of PGY-2 Pharmacy Residency in Pharmacy Leadership and Administrion.
