The role involves providing comprehensive pharmaceutical care to patients within an inpatient setting, ensuring safe and effective medication management, and collaborating closely with healthcare teams to optimize patient outcomes.
Key Responsibilities:
Review and verify inpatient medication orders for accuracy and appropriateness Dispense medications in compliance with hospital policies and regulatory requirements Monitor patient drug therapies for efficacy and safety, adjusting as necessary Provide expert drug information and consultation to healthcare providers and patients Participate in pharmacy rounds and contribute to interdisciplinary care teams Maintain accurate records and manage inventory to ensure availability of medications Support quality improvement initiatives related to medication use and safety
