POSITION SUMMARY
To be fully engaged in providing Quality/No Harm, Customer Experience, and Stewardship by providing comprehensive pharmaceutical care through integrated drug distribution, clinical pharmacy services and teaching under the direction of pharmacy residency preceptors. This position is part of a 12-month long, structured, post-graduate education and training program. The resident provides services consistent with other services provided by pharmacists at Health First. The pharmacy resident also provides comprehensive pharmaceutical care, clinical services and education.PRIMARY ACCOUNTABILITIES
Quality/No Harm:
Act ethically in the conduct of all pharmacy practice activities Maintains confidentiality in accordance with hospital policy and HIPPA rules and regulations Display initiative in preventing, identifying, and resolving pharmacy-related patient care problems Prepare and dispense medications using appropriate techniques and following Health First policies and procedures Assure appropriateness of a medication order, including age-related factors, before preparing or dispensing medications Determine and resolve any medication therapy problems including drugs without indications, medical conditions for which no therapy is prescribed, therapy inappropriate for medical condition, incomplete immunization record, inappropriate dose, dosage form, duration, schedule, route of administration, method of administration, therapeutic duplication, medication allergies, and clinically significant drug-drug, drug-disease, drug-nutrient, or drug laboratory test interactions. Participate in multidisciplinary patient care rounds Appropriately document clinical interventions Prevents and monitors for medication errors Detects, monitors, documents and reports adverse drug reactions (ADRs) and medication errors. Meets all the program requirements of a resident as outlined in the program handbookCustomer Experience:
Provide medication use education to patients and caregivers Counsels patients on admission or discharge medications as necessary Provide concise, applicable, and timely responses to requests for drug information from health care providers and patients Ensures pharmacy services are provided appropriately and in a timely manner Participates in the education and orientation of pharmacy, nursing, medical and other staff members Precepts pharmacy students consistent with academic credentials and clinical experience Provides presentations, publications, and other informative activities on drug-related topics to the health care community and general publicStewardship:
Participates in performance improvement activities and initiatives Provides cost-effective drug therapy alternatives to health care providers when appropriate Promotes the use of formulary medications by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary medications if necessary Identify and implement drug therapy where positive pharmacoeconomic impact could be achieved Develop and complete cost containment initiatives (e.g. MUEs) and report results to the Pharmacy and Therapeutics Committee Participate in formulary management and review of medication use processes through involvement in the Pharmacy and Therapeutics Committee Promote the use of the formulary by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary drugs when necessary Monitor the use of expensive medications to assure there is use consistent with approved criteria Work ExperienceQUALIFICATIONS REQUIRED
Graduate of an accredited college of pharmacy in the United States Licensed or eligible for licensure as a pharmacist in the state of Florida Licensure must be obtained with 90 days of starting the residency program Applications are accepted until January 10th each year through PhORCAS (Pharmacy Online Residency Centralized Application System) Health First Holmes Regional Medical Center participates in the ASHP Resident Matching Program for the selection of residents into the program Applications may be accepted in late March for any positions not filled during the matching processPHYSICAL DEMANDS
Minimum of 40 hours per week however, additional hours may be required Work hours may be long and irregular Prolonged periods of sitting, standing or walking Lifting/carrying moderate loads (up to 40 pounds) Repetitive use of hands and fingers Potential exposure to noise of varying intensitiesMENTAL DEMANDS
