Job Summary and Qualifications HCA Florida Fawcett Hospital is a Comprehensive Stroke Center. The Advanced Clinical Pharmacist will focus on critical care and will coordinate and assist with the development, implementation and maintenance of clinical pharmacy services, including but not limited to trauma/surgical, medical, neuro, and cardiovascular surgery intensive care services lines. Practice and adhere to the "Code of Conduct" philosophy and "Mission and Value Statement". What you will do in this role:
Environment:
Hospital intensive care units (ICU, CVICU).Multidisciplinary Team Participation:
Actively take part in daily medical rounds, providing real-time recommendations on drug selection, dosing, and monitoring. Participate in the training and preceptorship of new pharmacists, pharmacy students, residents, technicians, nurses and physicians.Medication Optimization:
Tailor medication regimens to complex, critically ill patients based on renal/hepatic function, hemodynamics, and PK/PD (pharmacokinetic/pharmacodynamic) principles. Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promoting drug therapy selection.Drug Monitoring & Safety:
Identify, prevent, and resolve adverse drug reactions and errors. Promotes antimicrobial stewardship in the critical care environment. Maintain an active role in the Medication Utilization Evaluation and Adverse Drug Reaction reporting. Maintain appropriate controlled substance records. Work with the Pharmacy Director and Manager to identify processes to meet or exceed all controlled substance diversion prevention and monitoring standards.Emergency Response:
Participate in code blue emergencies, preparing and advising on emergency medications.Protocol Development:
Develop and maintain intensive care unit protocols for areas like sedation, anticoagulation, and infectious diseases. Participates in the evaluation and analysis of clinical data, medication use, drug and drug class monographs, policy review, and order set development in support of clinical toolkit maintenance. Assist the Pharmacy Manager and Director in developing the clinical goals of the Pharmacy department.Patient Education & Transitions:
Provide education to patients and caregivers and manage medication reconciliation upon admission and discharge. Supports staff in the Pharmacy department and provides help and assistance when possible.What qualifications you will need:
Education:
Doctor of Pharmacy (PharmD) degree. Current Florida Pharmacist License or License eligible.Training:
3-5 years of experience in critical care. Postgraduate Year 1 (PGY1) pharmacy residency and/or a Specialized Postgraduate Year 2 (PGY2) Critical Care residency preferred.Certification:
Board Certified pharmacotherapy (BCPS) and/or Critical Care Pharmacist (BCCCP) preferred.Skills:
Customer service abilities including effective listening skills. Strong knowledge of critical care therapeutics, excellent communication skills, ability to work under pressure, and strong interpersonal skills for team collaboration. Critical thinking skills, decisive judgment and the ability to work with minimal supervision in a fast-paced environment.
