RESPONSIBILITIES I. JOB SUMMARY/RESPONSIBILITIES
Implements, maintains and provides clinical pharmacist services including: clinical, educational and informational functions for newborn, infant, pediatric, adolescent, adult and geriatric patients. Visits and provides clinical pharmacist services to patients in the community.II. TYPICAL PHYSICAL DEMANDS
Standing, sitting, stooping, bending, walking, finger dexterity, seeing, hearing, speaking.Frequent:
pushing/pulling usual weight of up to 40 pounds of force, lifting or carrying up to 30 pounds, reaching above, at and below shoulder level, frequent gripping of an object.Continuous:
repetitive arm/hand motions.Occasional:
kneeling, crawling, climbing stairs, walking on uneven ground, squatting, static gripping of an object for prolonged periods, pushing/pulling up to maximum 50 pounds.III. TYPICAL WORKING CONDITIONS
Not substantially subject to adverse environmental conditions. May be exposed to communicable diseases. Must have access to transportation.IV. MINIMUM QUALIFICATIONS
A.EDUCATION/CERTIFICATION AND LICENSURE
Current Hawaii State license as a Pharmacist. Doctor of Pharmacy degree or Bachelor of Science in Pharmacy degree. Post-graduate clinical residency accredited by the American Society of Health-System Pharmacists preferred. Board certification preferred. Current certification as a Certified Diabetes Care and Education Specialist (CDCES) preferred. Current BLS certification preferred.B. EXPERIENCE
