Job Description:
We are seeking a dedicated Clinical Pharmacist to provide safe, effective, and patient-centered medication therapy within a hospital setting. The ideal candidate will collaborate with healthcare teams, support clinical pharmacy services, and contribute to medication safety and quality improvement initiatives.
Responsibilities:
Provide direct patient-centered pharmaceutical care and medication therapy management. Collaborate with physicians, nurses, and healthcare professionals to optimize patient outcomes. Review medication therapies and identify potential drug-related issues. Participate in pharmacy operations, medication dispensing, and clinical programs. Support medication safety, stewardship, and quality improvement initiatives. Provide medication education and serve as a resource for healthcare teams. Support pharmacy students and staff through training and mentorship.
Must Have:
Licensed in Iowa as a Registered Pharmacist. Graduate of a college of pharmacy. Clinical pharmacy experience (2-4 years) in a hospital setting required. Strong communication and critical thinking skills. Ability to work independently and collaborate with healthcare teams.
Preferred Qualifications:
PharmD degree preferred. Completion of ASHP-accredited PGY-1 Pharmacy Residency preferred. Board Certification in Pharmacotherapy Specialties (BCPS) preferred. Knowledge of contemporary hospital and clinical pharmacy practice. Required Experience / Licenses /
Certifications:
Minimum Experience:
2-4 years of clinical pharmacy experience in a hospital setting.
Required License:
Licensed in Iowa as a Registered Pharmacist.
Required Education:
Graduate of a College of Pharmacy.
Preferred Certifications:
