PM Summary:
Provides medication management services as part of an interdisciplinary team that ensures safe, efficacious, and cost-effective care. Involved in operational duties including order verification, oversight of sterile and non-sterile preparations, and medication distribution. Provides clinical services including multidisciplinary rounds, pharmacy consults, and quality initiatives. Acts as an information resource to patients, providers, nurses, and other healthcare professionals regarding medication therapy and drug-related information. Responsible for participation in and completion of all patient safety initiatives appropriate to the position. May have access to highly confidential patient, employee and/or Organizational proprietary information, and must handle and protect the information in accordance with hospital and system policies, HIPAA requirements and the highest level of ethical standards. Serves as a preceptor to students, new hires, and residents. Provides oversight, guidance, and verification of work completed to pharmacy technicians.
Responsibilities:
+ Participates in precepting/training of pharmacy students, residents, pharmacy technicians and new hires. + Participates in patient counseling and multidisciplinary rounds where applicable. + Provides drug information for providers, nursing, and other healthcare professionals. + Performs pharmacotherapy related interventions to optimize patient care. + Verification of medication orders for clinical appropriateness and accuracy. + Ensures quality and accuracy of sterile and non-sterile compounding pharmacy activities. + Provides oversight of pharmacy technicians and medication distribution. + Proactive in identifying problems and solutions within the department to help improve patient care and patient safety. + Other Duties.
