8:00 AM•1:00 PM Weekend and On-Call rotation required Job Summary Seeking an experienced Inpatient Pharmacist to support hospital pharmacy operations in an acute care setting. Responsibilities include medication order verification, clinical monitoring, pharmacokinetic dosing, sterile compounding, interdisciplinary collaboration, medication reconciliation, and supervision of pharmacy support staff. The pharmacist will also assist with patient counseling and ensure safe and effective medication management. Requirements Requirements Minimum 2 years of inpatient pharmacist experience required Active Michigan Pharmacist License required at time of submission Experience working in an Anticoagulation Clinic required Must be comfortable managing 4-6 anticoagulation clinic patients daily Oncology compounding experience required Experience performing IV compounding independently required Strong knowledge of sterile compounding procedures and medication safety practices preferred Ability to collaborate effectively with physicians, nurses, and healthcare staff required Key Responsibilities Review and verify medication orders for accuracy and safety Monitor patient medication profiles, allergies, lab values, and drug interactions Perform pharmacokinetic dosing and clinical interventions Prepare and oversee IV medications, TPN, chemotherapy, and sterile compounds Provide medication reconciliation and discharge counseling Supervise pharmacy technicians, interns, and support staff
