The Clinic Pharmacist is a key member of the LifeCircles PACE team, delivering comprehensive, participant-centered clinical pharmacy services that optimize medication therapy and improve health outcomes for older adults. Working collaboratively with the Director of Clinical Services, Medical Director, Primary Care Providers, the Interdisciplinary Team (IDT), contracted pharmacies, hospitals, and post-acute care facilities, this role ensures safe, effective, and evidence-based medication management while preventing gaps in care during transitions. The Clinic Pharmacist provides expert clinical guidance to identify and resolve medication-related concerns, oversees medication utilization and pharmacy benefit management (PBM) initiatives, promotes compliance with Medicare Part D regulations, and champions cost-effective prescribing strategies. This position will work between the Holland and Muskegon locations.
