Clinical Pharmacist I - Float (Full-Time) No Weekends or Holidays | $10,000 Sign-On Bonus (Paid at 6, 12, 18, and 24 months) Position Summary The Clinical Pharmacist I collaborates with an interdisciplinary healthcare team to ensure safe, effective, and evidence-based medication therapy. This role serves as a key clinical resource for physicians, nurses, pharmacy staff, and patients, while supporting high-quality patient care aligned with organizational mission, values, and service standards. Key Responsibilities Clinical Practice & Patient Care Review patient records to evaluate the appropriateness and effectiveness of medication therapy Assess patient conditions to ensure comprehensive treatment of all clinical concerns Develop and optimize pharmacotherapy plans to improve outcomes and minimize adverse effects Monitor drug therapy through chart review and interdisciplinary collaboration; intervene when appropriate Consult with providers, nurses, and patients on medication selection, dosing, administration, interactions, and side effects Evaluate treatment outcomes and adjust therapy recommendations as needed Maintain accurate and timely documentation of patient care activities Provide Medication Therapy Management (MTM) services, including dose, route, schedule, contraindication, and interaction assessment Collaboration & Communication Work closely with physicians, nurses, and allied health professionals to support patient-centered care Serve as a drug information resource for clinical staff and patients Maintain strong professional relationships across care teams Communicate effectively across shifts to ensure continuity of care Operational & Quality Responsibilities Evaluate medication orders for clinical appropriateness and operational efficiency Participate in quality assurance initiatives, including adverse drug reaction reporting and medication use evaluations (MUEs) Assist with departmental initiatives such as policy development and review Ensure compliance with regulatory, licensing, and accreditation standards Participate in investigational drug protocols as assigned Education & Professional Development Precept pharmacy students and residents; support training and competency development Provide in-services, presentations, and continuing education to staff Maintain current licensure and continuing education requirements Participate in committees and professional development activities as needed Serve as a clinical resource and maintain expertise within designated specialty areas Safety & Compliance Adhere to all confidentiality and patient privacy policies Complete required safety training, inspections, and compliance documentation (e.g., fire/safety, expiration checks, floor stock reviews) Uphold infection control standards, PPE requirements, and organizational policies Technology & Systems Demonstrate proficiency with pharmacy systems (e.g., Epic) for order verification and clinical documentation Utilize drug information databases (e.g., Micromedex) effectively Operate pharmacy automation systems (e.g., Omnicell, ScriptPro, McKesson, Baxa) as applicable Professional Standards Deliver exceptional service using organizational communication and customer service standards Demonstrate professionalism, teamwork, and accountability in all interactions Maintain professional appearance and adhere to dress code guidelines Effectively manage time and prioritize responsibilities in a fast-paced environmentQualifications Education:
Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited school of pharmacyLicensure:
Active pharmacist license in the state of practiceExperience:
