Job Description
Job Overview We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for patients. This position requires a strong background in clinical pharmacy, patient care, and the ability to work collaboratively within a multidisciplinary team. The Pharmacist will be responsible for medication management, patient assessments, and providing exceptional patient service in a dynamic healthcare environment. Responsibilities Review and interpret medication orders to ensure accuracy and safety. Provide clinical pharmacy services including patient assessment and medication therapy management. Collaborate with healthcare professionals to optimize patient care plans. Monitor patient responses to medications, adjusting therapies as necessary. Educate patients and healthcare staff on medication use, side effects, and interactions. Prepare and dispense medications, including IV infusions, adhering to aseptic techniques. Maintain accurate records of medication administration and patient care activities. Participate in hospital committees related to pharmacy services and medication safety. Stay current with medical terminology, physiology, anatomy knowledge, and advancements in pharmacy practice. Skills Proficient in managed care principles and practices. Experience in Level I or Level II trauma centers is preferred. Strong background in hospital medicine and critical care experience. Completion of a pharmacy residency program is highly desirable. Clinical pharmacy experience with a focus on acute care settings such as ICU or PICU. Knowledge of pain management strategies and acute pain management protocols. Familiarity with emergency room operations and medication administration processes. Excellent communication skills for effective patient service and collaboration with healthcare teams. Strong understanding of medical terminology relevant to pharmacy practice.
