Job Description:
Elliot Hospital Pharmacy | 1 Elliot Way Manchester NH Clinical Pharmacist Per Diem, variable shifts. Elliot Hospital Inpatient Pharmacy The inpatient pharmacy department is comprised of pharmacists and technicians working collaboratively to provide safe, timely and effective medication therapy to our patients. We operate a decentralized model for both medication distribution utilizing Omnicell automation, as well as clinically with pharmacists dedicated to the teams on our nursing units. Elliot promotes career advancement through our ASHP accredited PGY-1 Pharmacy Residency Program, technician career ladder, and reimbursement for a variety of professional certifications. About the Job The Clinical Pharmacist operates in both central distribution and decentralized nursing unit settings. The Clinical Pharmacist leverages strong clinical aptitude along with a team-first attitude to positively affect patient outcomes in a multidisciplinary health care team environment.
What You'll Do Medication Order Processing:
Prompt and accurate medication order processing of computer physician order entry (CPOE) via the electronic health system (EHR).
Clinical Pharmacy:
Monitors drug therapy to prevent contraindications, duplicate therapy, drug interactions and allergic reactions. Recommends appropriate medication doses and adjustments as indicated by patient conditions. Responds to drug information requests with accurate, clinically relevant, reliable, and valuable information in a timely manner. Collaboration with
Healthcare Team:
Participate in multidisciplinary team rounding with physicians, physician assistants, nurse practitioners, nursing, case management and other disciplines involved in patient care.
Patient interactions:
Ability to interact with patients and caregivers including accurate collection of patient's medication history and provide education when appropriate. Respond to critical patient situations (i.e.. Traumas, codes) to provide pharmacy support as part of the medical response team.
Education:
Provides training and education to pharmacists, nurses, providers, students, and residents as applicable.
Development:
Actively involved with personal professional development and growth to support clinical care of patients. Shows engagement with and completion of education and training beyond licensure requirements. Acts to improve work and departmental performance.
Pharmacy Operations:
Understands and adheres to hospital formulary whenever possible. Provides recommendations for therapeutic interchange as necessary. Participates in medication utilization or other assessments of clinical, cost-effective use of medications. Oversight of technician duties, ensuring work for accuracy. Understands, supports, and organizes technician workflows to meet patient needs and improve patient care. Provides leadership and direction in oversight responsibility with technicians. What You'll Need PharmD or BS in Pharmacy Required. Current New Hampshire Pharmacist license to practice Pharmacy required. Board of Pharmacy Specialist (BPS) cert strongly preferred. Residency preferred. Basic Life Support Certification preferred but not required. One (1) year previous pharmacy experience preferred. Epic Willow experience preferred. Knowledge of New Hampshire drug laws preferred.
