Mid-Level (2 5 years hospital experience) Guaranteed Hours Full-time (40 hours per week) Estimated Salary Range $123,000 $184,500 per year Job Overview We are seeking a Clinical Pharmacist for an inpatient hospital pharmacy setting within a rural acute care facility. The ideal candidate will be responsible for ensuring safe, effective, and evidence-based medication therapy management across a wide range of clinical services. This role involves direct interaction with physicians, nursing staff, and interdisciplinary teams to optimize medication use, support patient safety, and ensure compliance with hospital protocols and regulatory standards. Key Responsibilities Medication Management & Dispensing Interpret and verify physician medication orders Compound and dispense medications including IV admixtures, chemotherapy, and parenteral nutrition Maintain accurate computerized patient medication profiles Ensure safe handling and dispensing of controlled substances Clinical Pharmacy Services Monitor drug therapy for interactions, contraindications, allergies, and dosing accuracy Perform pharmacokinetic dosing per hospital protocol Review lab and culture data to optimize antibiotic therapy Provide clinical recommendations and therapeutic alternatives to providers Document clinical interventions and adverse drug reactions Patient Safety & Compliance Ensure compliance with federal, state, and hospital regulations Participate in medication safety, quality improvement, and audits Maintain formulary compliance and support therapeutic substitution protocols Assist in Code Blue / emergency response situations Collaboration & Education Provide drug information to healthcare staff, patients, and families Participate in staff training, orientation, and education programs Collaborate with physicians, nurses, and interdisciplinary teams

Hospital-based inpatient pharmacy schedule Requirements Graduate of ACPE-accredited School of Pharmacy (BS or PharmD) Active Colorado Pharmacist License or eligibility required 2 5 years hospital pharmacy experience required Experience with medication reconciliation and drug therapy management Experience or familiarity with Code Blue response preferred BLS (AHA) required ACLS (AHA) required or preferred per hospital standards
