Job Description - Clinical PharmacistPosition Overview A reputable healthcare facility in Saranac Lake, New York is seeking an experienced Clinical Pharmacist to join its Pharmacy Services team on a full-time basis. This position is responsible for providing comprehensive clinical pharmacy services, ensuring the safe and effective dispensing of medications, collaborating with healthcare providers, and supporting optimal patient outcomes. The ideal candidate will have strong hospital clinical pharmacy experience and an active New York Pharmacist license.

Saranac Lake, New York Key Responsibilities Provide clinical pharmacy services to optimize medication therapy and improve patient outcomes. Review, verify, and accurately dispense medications in accordance with physician orders and departmental policies. Evaluate medication appropriateness, dosing, drug interactions, allergies, contraindications, and therapeutic alternatives. Monitor patient medication therapy and recommend adjustments when clinically appropriate. Collaborate with physicians, nurses, and other healthcare professionals to ensure safe and effective medication management. Provide drug information and clinical consultation to medical staff and other healthcare providers. Counsel patients and caregivers regarding medication usage, side effects, and adherence when appropriate. Ensure compliance with all federal, state, hospital, and departmental pharmacy regulations and standards. Maintain accurate documentation and medication records. Participate in quality improvement initiatives, medication safety programs, and pharmacy operations. Support inventory management and ensure the proper storage and handling of medications and pharmaceutical supplies. Required Qualifications Registered Pharmacist currently licensed in the State of New York . 2-3 years of clinical pharmacy experience is required. Hospital pharmacy experience is required. Strong knowledge of clinical pharmacy practices, medication management, and pharmaceutical care. Excellent communication, interpersonal, and collaboration skills. Ability to work effectively in a multidisciplinary healthcare environment. Strong attention to detail and commitment to patient safety. Preferred Qualifications Experience providing direct clinical pharmacy services in an acute care hospital. Experience collaborating with physicians and multidisciplinary care teams. Familiarity with electronic health records (EHR) and computerized physician order entry (CPOE) systems. Strong clinical judgment and evidence-based decision-making skills.
