Job Description
About the Role We are seeking a highly skilled Clinical Pharmacist II to join our hospital‑based pharmacy team. This is an onsite, decentralized clinical role supporting interdisciplinary care across assigned units. The pharmacist will work autonomously with providers, nurses, and patients to optimize medication therapy, support safe medication use, and advance clinical pharmacy practice. Only candidates who hold a PharmD and have 1-3 years of hospital pharmacy experience will be considered. Key ResponsibilitiesClinical Practice Provide advanced pharmaceutical care within assigned specialty units Participate in multidisciplinary rounds and collaborate with care teams Perform medication reconciliation and patient counseling Deliver drug information and education to patients and providers Manage complex medication regimens and resolve therapy‑related issues Medication Verification & Dispensing Independently verify medication orders for accuracy and appropriateness Dispense medications per hospital policies and safety standards Maintain high accuracy and efficiency in medication distribution Patient Monitoring Review medication profiles for interactions, allergies, contraindications Identify and resolve drug‑related problems Support safe and effective medication use across the continuum of care Residency Program Support Precept PGY1 pharmacy residents Provide education, mentorship, and clinical supervision Contribute to curriculum development and educational presentations Sterile Compounding Follow
