Job Description

Job Summary :
The Medication Therapy Management (MTM) Pharmacist is responsible for delivering comprehensive medication management services to improve patient outcomes, reduce medication-related problems, and support valueâ??based care initiatives. The pharmacist works directly with patients, providers, and care teams to identify medicationâ??related issues, optimize therapy, ensure adherence, and promote safe, evidenceâ??based medication use.
Responsibilities:
Provide Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs) via phone, telehealth, or inâ??person. Identify and resolve medication-related problems, including efficacy, safety, adherence, cost, contraindications, and duplications. Provide patient education on medication therapy, disease states, lifestyle modifications, and self-management strategies. Develop individualized medication action plans and share follow-up recommendations with providers. Ensure appropriate documentation in MTM platforms (e.g., OutcomesMTM, Mirixa/DecisionRx, internal EHR). Support transitions of care to prevent readmissions and improve continuity.
Qualifications/Requirements:
Experience:
1-3 years of experience in ambulatory care, community pharmacy, MTM, or clinical pharmacy practice. Strong knowledge of chronic disease management, guidelines, and MTM platforms. Excellent communication and patient engagement skills
Education:
PharmD required; advanced degree (MS, MHA, MBA, or MPH) preferred.. Licenses /
Certifications:
Current NYS pharmacist license required, or eligible to obtain within 90 days of hire.
