Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A home infusion clinical pharmacist plays a crucial role in managing the preparation, dispensing, and monitoring of complex intravenous (IV) and subcutaneous medications for patients receiving care at home. This position demands clinical expertise and collaboration with a multidisciplinary team to ensure safe and effective therapy while providing essential education and support to patients and caregivers.
Responsibilities:
Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment, adhering strictly to state and federal regulations. Assess lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) to adjust treatment plans accordingly. Provide comprehensive patient counseling and education regarding specialty medications, infusion pump use, sterile techniques, and catheter maintenance. Collaborate with prescribing physicians, home health nurses, and internal teams—including intake, referral, reimbursement, nursing, clinical services, and pharmacy operations—to ensure seamless therapy management and prevent hospital readmissions. Maintain compliance with regulatory requirements, including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant standards. Oversee pharmacy inventory management, including refrigerated products, and coordinate packing and shipping of medications to patients' homes.
Qualifications and Desired Experience:
Current and good standing Ohio pharmacist license. Minimum 2 years of experience in infusion specialty pharmacy. Experience with IVIg therapies is a plus. Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations. Proficiency with CareTend system preferred.
