Job Description

POSITION SUMMARY
The Consultant Pharmacist functions as an integral part of the pharmacy team and is directly responsible for reviewing and monitoring residents' medication regimens on a scheduled basis for clinical appropriateness and compliance with regulations. This individual uses best practices to maintain or improve delivery, storage, and administration of medications. The Consultant Pharmacist interacts professionally and develops relationships with facility administration, providing reporting resources and clinical expertise to support compliance with policies and procedures that positively affect patient care and facility operations.
POSITION STATUS
Hourly, Non-Exempt (Part-time) OR Salary, Exempt (Full-time)
PRIMARY DUTIES AND RESPONSIBILITIES
Shall include but not be limited to the following: Resident medication regimen reviews: Scheduled Interim/Change-in-Condition upon request Documenting and tracking recommendations and follow-up to support regulatory/survey compliance Attend and prepare reports for Quality Assurance (QA) Meetings or other facility committees Attendance at monthly consulting meetings and implementing/engaging in the team's clinical initiatives Medication Pass Observations/Inservices Vaccination clinic support Oversees and supports controlled medication accountability systems Supports facility education: Policies and Procedures Formulary Clinical Initiatives/Program Offerings (i.e. Antimicrobial Stewardship)
EXPECTATIONS OF EMPLOYEE
Ohio Pharmacist license in good standing Valid driver's license Minimum of 2 years experience working in long-term care pharmacy Board Certified Geriatric Pharmacist (BCGP) and/or Age Friendly designation preferred Ability to represent Absolute Pharmacy in a positive and professional manner Communicates regularly and constructively with peers, managers and facility personnel Demonstrates flexible and efficient time management and ability to prioritize workload with minimal supervision Excellent communication, interpersonal and organization skills Able to travel to facilities on a scheduled and as needed basis
