Job Description

SSM Health seeks a Pharmacist Clinical Specialist to deliver advanced, evidence-based medication management in a collaborative, patient-centered environment. In this role, you will provide direct clinical pharmacy services, optimize complex drug therapies, participate in interdisciplinary rounds, and serve as a medication expert for patients and the care team. You will support protocol development, quality initiatives, and education for staff and learners while practicing within SSM Health's mission-driven culture of compassionate service, respect, and professional growth. Responsibilities Provide evidence-based clinical pharmacy services to optimize medication therapy outcomes Collaborate with physicians, nurses, and care teams on individualized patient care plans Review, verify, and adjust medication orders for safety, efficacy, and cost-effectiveness Participate in patient care rounds and provide drug information and therapeutic recommendations Educate patients and families on medication use, side effects, and adherence Precept and mentor pharmacy residents, students, and new pharmacists Develop and implement clinical protocols, order sets, and best practice guidelines Monitor and report medication-related quality metrics and participate in performance improvement initiatives Ensure compliance with regulatory, accreditation, and institutional medication use policies Contribute to on-call and weekend clinical pharmacy coverage as needed Required Skills Clinical pharmacotherapy expertise Medication therapy management (MTM) Order verification and medication safety Interdisciplinary care collaboration Patient counseling and education Protocol and guideline development Therapeutic drug monitoring (e.g., vancomycin, anticoagulants) Use of electronic health records and CPOEData analysis and quality improvement Precepting and clinical teaching
