Full Job Description Pharmacist Schedule:
Monday-Friday, 9:00 AM-4:00 PM Job Summary We are seeking a licensed Pharmacist responsible for safely dispensing medications, monitoring patient medication therapy and compliance, providing medication counseling, and collaborating with physicians, nurses, pharmacists, and other healthcare professionals to optimize patient care. Key Responsibilities Accept, review, prepare, and dispense prescription medications in accordance with applicable laws and regulations. Monitor medication compliance, overuse patterns, drug interactions, and potential drug-related problems. Counsel patients on medications, including dosage, administration, storage, side effects, and interactions. Provide consultation on prescription and non-prescription medications. Maintain accurate, confidential patient records and pharmacy files. Compound medications for internal and external use and prepare sterile solutions using appropriate procedures. Evaluate and monitor drug therapies for home-care patients and identify potential drug-related issues. Provide patient services such as smoking cessation, blood pressure measurement, and cholesterol management. Educate patients on proper use of medical devices, including blood pressure monitors and glucose meters. Supervise pharmacy technicians, clerks, and other pharmacy staff as required. Maintain appropriate inventory of medications, pharmaceutical supplies, and medical supplies. Communicate effectively with physicians and other healthcare practitioners regarding patient drug therapy. Maintain compliance with pharmacy laws, policies, procedures, and applicable standards. Required Qualifications Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) preferred/required per state or facility requirements. Active Registered Pharmacist (RPh) license in the state(s) of practice. Experience with compounding infused drugs and sterile preparations. Proven experience evaluating and monitoring drug therapies for home-care patients. Strong organizational and time-management skills. Excellent verbal and written communication skills. Professional society membership/participation is desirable.
Pay:
