Job Description
Providence Hood River Memorial Hospital is seeking an experienced Inpatient Pharmacist with a strong background in Oncology and Outpatient Infusion Services . This position supports inpatient and ambulatory care, providing clinical pharmacy services, chemotherapy verification, infusion medication management, and interdisciplinary patient care. Key Responsibilities Verify and review hazardous and non-hazardous outpatient infusion medication orders , including chemotherapy. Evaluate oncology and non-oncology treatment plans for safety and appropriateness. Perform IV infusion compounding verification and medication preparation oversight. Monitor pharmacotherapy, therapeutic drug levels, and high-alert medications. Provide infusion medication education and counseling to patients. Participate in the management of infusion-related adverse reactions. Verify insurance authorization and treatment plan approvals prior to therapy initiation. Collaborate with physicians, nurses, and the multidisciplinary care team to optimize medication therapy. Utilize Epic and Pyxis systems for medication management and documentation. Requirements Minimum 1 year of outpatient infusion pharmacy experience (required). Oncology pharmacy experience required. Active Oregon Pharmacist License required.
