7:30 am - 6:00 pm, Monday, Tuesday, Thursday, Friday Your role with us Pharmacists at Salem Health's Medication Management clinic optimize patient medication therapy by collaborating with healthcare teams to interpret treatment goals, address complex medication issues and improve outcomes. The role includes providing clinical services such as collaborative drug therapy management, patient counseling, and education for healthcare professionals, promoting medication safety and best practices. What you'll do Interprets treatment goals and makes recommendations on medication orders/prescriptions to ensure treatment plans are complete, safe and aligned with institutional policy and best practices. Participates in medication prescribing under Collaborative Practice Agreements with Salem Health providers for chronic disease states and complex patients. Provides collaborative drug therapy management services, including physical assessments, motivational interviewing and patient counseling in ambulatory care clinics. Provides drug monitoring services and assists with navigating prior authorizations as needed. Interprets policies and procedures for hospital and clinic staff. Assesses and resolves educational needs of healthcare professionals, including providers, nurses, and allied health staff, on medication therapy management. Required qualifications Doctor of pharmacy degree or a bachelor of science in pharmacy with applicable work experience at the discretion of the hiring manager. State of Oregon pharmacist licensure. Board Certified Pharmacotherapy Specialist (BCPS) certification within two years from date of hire. Basic Life Support (BLS) certification obtained within 90 days from date of hire. PGY-1 residency or four years of previous ambulatory care/clinical patient care practice, along with prior direct experience in patient or customer service interaction in-lieu of residency experience. Additional information This role is best suited for individuals with a strong background in ambulatory care, ideally complemented by a PGY-2 residency. Proficiency in Epic or similar electronic health record systems enables efficient workflow management and supports optimal patient care, making it a key component of success in this role.
