- Ambulatory Care and experience with chronic disease management will be crucial to success (i.e. diabetes, hypertension, dyslipidemia).
- Experience in 340B Responsibilities The Ambulatory Clinical Pharmacist is responsible for providing patient care and advanced clinical pharmacy services in the ambulatory pharmacy space, including referral based HOPD pharmacist managed clinics, specialty pharmacy, and population health pharmacy services.
